I certify that I am an adult authorized to sign this Release on my own behalf, and on behalf of all minors and all adults participating in Invicta Sports Club activities under my reservation.
I understand that the sport of archery and/or fencing involve inherent risks that include the risks of injury, accident, or disability. Intending to be legally bound, I hereby, for myself, my heirs, executors, administrators, and assigns, release and forever discharge Invicta Sports, Z-Parta LLC, and all of their owners, coaches, instructors, and assistants, officers, employees, agents, and representatives for, from, and against any and all claims, rights, and suits for damages relating to any injuries incurred by me and/or all minor children and adults participating in activities under my reservation, as a result or in the course of participation in the Invicta Sports activities at Invicta Sports facilities.
I am, and/or all minors and adults participating in activities under my reservation are in good physical condition and have/has no underlying physical problems that would preclude my/his/her participation in Invicta Sports activities. Invicta Sports personnel have no expertise in treating any medical conditions; I hereby consent to Invicta Sports requesting and obtaining medical care at my expense from any licensed physician, hospital, and/or clinic for any injury or illness that may arise during activities at Invicta Sports facilities.
I affirm that I agree to abide by the rules and guidelines of the Invicta Sports program. I have been given an opportunity to ask any question that I may have. I have fully informed myself of the contents of this release form by reading it before signing.
